The Contact Form Page is where you will collect the respondent's contact details and ask for their consent.
To edit this page, go to Contact Form Page.
You can add the requirements for their contact details as well as add in custom fields. You can also add consent text and edit the page content.
Contact Details
In this tab you can toggle the information you would like to collect on or off.
These include:
Collect contact details - If this is toggled off, all the fields will disappear.
Collect at the start - You can toggle this on to move the process to the beginning. So for example, capture their details before they capture any testimonials for you.
Name - If this is toggled on they will need to add their name. You can make this required and add placeholder text.
Email address - If this is toggled on they will need to add their email address. You can make this required and add placeholder text.
Job Title - If this is toggled on they will need to add their job title. You can make this required and add placeholder text.
Star rating - This will allow your customers to give you a star rating. See here for more information about adding a star rating and add placeholder text.
Additional photos and videos - If toggled, the respondent will be able to upload photos and videos to be used as b-roll in the final video, add placeholder text.
Headshot - This is an image of the respondent and is ideal for text replies, add placeholder text.
Company - The respondent can add their company name, add placeholder text.
Phone Number - The respondent can add their phone number, add placeholder text.
You will then be able to find the respondent's contact details in the individual reply after clicking on their name.
