How to Edit Your Contact Form Page

The Contact Form Page is where you will collect the respondent's contact details and ask for their consent.

To edit this page, go to Contact Form Page.

Contact Details

In this tab you can toggle the information you would like to collect on or off.

These include:

  • Collect contact details - This includes email and name.

  • Additional photos and videos - If toggled, the respondent will be able to upload photos and videos to be used as b-roll in the final video.

  • Headshot - This is an image of the respondent and is ideal for text replies.

  • Star rating - This will allow your customers to give you a star rating. See here for more information about adding a star rating.

  • Title - The respondent can add their job title.

  • Company name - The respondent can add their company name.

  • Phone Number - The respondent can add their phone number.

Custom Fields

You can also add in custom fields if there is specific information you would like to capture.

To add a custom field, go to + Add custom field.

You can then add in the following information:

  • Field name - This is the name of the field so for eg, when did you first sign up?

  • Field type - You can choose a field type from the dropdown menu. These include single or multi-line text input, checkbox, date picker, file attachment, number input and more.

  • Label text - This is the text that will be inside the input area.

  • Required - Here you can toggle if you want the field to be required or optional.

Once you have added in your fields, go to Save field and toggle in on to see it on your form preview on the right.

Get Consent

This is where you can ask for the respondent's consent to use their content. You can toggle this on or off.

If this is toggled on, you can customize the consent text and make it mandatory if you'd like. If you choose not to customize the consent text, it will remain as our default text.

The respondent will then have to check whether they give consent.

Page Content

In this tab you can edit the headline of the contact page and add your form instructions.
